Submit the Service Member’s Group Life Insurance Premium Reimbursement Application with a copy of your orders and your Leave and Earnings Statements for the months you are claiming, following the submission route in DMNA Regulation 1-2 for your type of service. Apply by 1 November each year for duty performed in the previous federal fiscal year.

• You are an active member of the New York National Guard
• You served on state active duty other than for training, federal active duty other than for training, Active Duty Operational Support, Active Guard Reserve duty while not mobilized, or a New York Air National Guard Military Personnel Appropriation tour
• You paid SGLI premiums for that period of service
