Skilled nursing care at four state homes

Four state skilled nursing homes serve veterans and their eligible family members: Salisbury with 99 beds, Black Mountain with 100, Kinston with 100 and Kernersville with 120. Care comes from registered nurses, licensed practical nurses and certified nursing assistants under licensed physicians, alongside a registered dietician, social workers, an on-site veterans service officer, physical, occupational and speech therapy, recreation and community programs, and hospital care through a VA medical center or a local hospital.

What you'll need

Your DD214 showing active duty service and a discharge under honorable conditions, proof that you have lived in North Carolina for the two years before admission, and a licensed physician’s referral confirming that you need skilled nursing care.

Eligibility

  • You served on active duty in the U.S. Armed Forces other than for training
  • You received a discharge under honorable conditions
  • You have been a North Carolina resident for the two years before your admission date
  • A licensed physician has referred you and determined that you need skilled nursing care
  • You are disabled by age, disease or another reason confirmed by a physical exam
  • The home can meet your care needs
