You can request your Ohio military service record, if you enlisted from Ohio, and Ohio wartime bonus records, which are no longer being added to. Requests go through your County Veterans Service Office.

Submit the Request for Military Service Record by mail to Request for Military Service Record, Ohio Department of Veterans Services, 77 S. High Street, 7th Floor, Columbus, OH 43215, or by fax to 614-387-7317. For help, call 888-387-6446 or contact your local County Veterans Service Office.

What you'll need

The completed Request for Military Service Record form and your identifying information, including the name you served under and your service dates.
