Gold Star plate in four family versions

Immediate family members of a service member killed while serving on active duty, or of an honorably discharged veteran who died from a service-connected injury, illness or disease, can get an Oklahoma Gold Star license plate. Four versions exist for different relatives: Gold Star Spouse, Gold Star Family, Gold Star Parent and Gold Star Survivor.

How to apply

Mail your first application and the documentation listed on page two of the form to Service Oklahoma, PO Box 26940, Oklahoma City, OK 73126-0940. Later renewals can be processed at a Service Oklahoma driving and auto location.

What you'll need

The documentation listed on page two of the Gold Star plate application, which covers proof of the death and proof of your relationship to the service member or veteran.

Eligibility

  • You are an immediate family member of a service member who was killed while serving on active duty, or of an honorably discharged veteran who died from a service-connected injury, illness or disease
  • You qualify for one of the four Gold Star plate versions
