Purple Heart recipients can get an Oklahoma Purple Heart license plate, and it is also made for motorcycles. The unremarried surviving spouse of a deceased Purple Heart recipient can apply for it too.

Apply at a Service Oklahoma driving and auto location, or mail your completed application and documentation to Service Oklahoma, PO Box 26940, Oklahoma City, OK 73126-0940.

What you'll need

Official documentation from the U.S. Armed Forces or the VA showing you were awarded the Purple Heart. A surviving spouse also needs the veteran's death certificate and a marriage certificate.

Eligibility

You received the Purple Heart, or you are the surviving spouse of a deceased Purple Heart recipient
If you are claiming as a surviving spouse, you have not remarried
