Your Social Security number, home and mailing address, phone number, a valid email address, your UC System username and password if you have one, your bank name, address, account and routing number if you want direct deposit, your last employer's name, address, phone number and Pennsylvania UC account number if you know it, your first and last day worked, your reason for leaving, and any pension or severance package information.
