Ask the Providence VA Regional Office for a War Related Disabled Declaration letter, then send that letter with the completed Disability Parking Placard application by fax to 401-462-0829, or by mail to the Rhode Island DMV, Attn: Disability Parking Placard Office, 600 New London Avenue, Cranston RI 02920-3024.
