Your state active duty orders, a line of duty determination, your medical records for the injury or illness, and a death certificate if you are a survivor filing a claim.

Eligibility

  • You are a Rhode Island National Guard service member serving on state active duty
  • You were injured, became ill or became disabled in the line of duty, or you are the survivor of a Rhode Island National Guard service member who died in the line of duty on state active duty
