Contact the Utah National Guard, which pays the gratuity to the service member’s named beneficiaries. The Utah Department of Veterans and Military Affairs can help at 801-326-2372.

What you'll need

The service member’s state active duty orders, the death certificate, the line of duty determination, and proof that you are a named beneficiary.

  • You are a beneficiary of a Utah National Guard service member
  • The service member died in the line of duty while serving on state active duty
