If you are a full-time active duty service member, the Utah Division of Professional Licensing will waive the fee on your initial professional licence application and on your renewals.
Submit the Military and Public Assistance Request for Fee Waiver form together with your initial or renewal licence application to the Utah Division of Professional Licensing.
The fee waiver request form and proof that you are serving full time on active duty, such as your orders or military ID.
