You can claim a Utah income tax credit on survivor benefits you receive after a service member’s death. The credit is your federally taxable survivor benefits multiplied by 0.0455, which is 4.55 percent.

How to apply

There is nothing to file in advance. Claim the credit on your Utah individual income tax return when you file.

What you'll need

Your Form 1099-R or award statement showing the survivor benefits, and documentation of the service member’s death.

Eligibility

  • You file a Utah individual income tax return
  • You are the spouse or dependent child of a service member who died while serving on active duty, or of a reserve component service member who died in the line of duty while on inactive duty for training
  • You receive survivor benefits that are included in your federal adjusted gross income
