It replaces part of your lost wages while you look for other work, is not based on financial need, and is paid for entirely by employers. Your Social Security number, the names of all your employers as they appear on your pay stubs with the dates you worked, the name and local number of your union hall if you have one, your driver licence or ID card number, your Alien Registration number if you are not a U.S. citizen, and the mailing address and phone number of any non-Utah employers.
