Three state medals recognising Vermont military service

Vermont awards three medals recognising the service and sacrifice of service members and their families: the Vermont Veterans Medal, the Vermont Distinguished Service Medal and the Vermont Patriots Medal. You can receive your medal by mail or at the annual presentation ceremony held by the Governor at the Vermont State House.

How to apply

Complete the Vermont State Veterans Medal and Distinguished Service Medal Application with the documentation listed on it, and mail or email it to the Vermont Office of Veterans Affairs, 118 State Street, Montpelier VT 05620-4401, or mil.ovainfo@vermont.gov. What you'll need

Your DD214 showing your service and honorable discharge, and proof of Vermont residency or Vermont unit assignment. Next of kin applying for a Patriots Medal need the casualty documentation and proof of relationship.

Eligibility

  • You are a Vermont veteran or service member, or the next of kin of a Vermont service member killed in action
  • You qualify for at least one of the three Vermont medals
