A copy of one of the following: your VA ID card or VA Health ID card, DD214, DD256, DD257 or NGB22.

You hold or are applying for a Kentucky driver’s license or ID card
You received an honorable discharge
You can show a VA ID card or VA Health ID card, DD214, DD256, DD257 or NGB22
