Report the injury or illness through your chain of command and file an industrial insurance claim with the Washington State Department of Labor and Industries.

What you'll need

Your state active duty or inactive duty orders, a line of duty determination, and your medical records for the injury or illness. Survivors will also need the death certificate.

Eligibility

  • You are a Washington State National Guard service member, or the survivor of one
  • You were injured, became ill or disabled, or died while serving on state active duty or inactive duty
