Submit the completed Application for Military Skills Test Waiver, including certification by your commanding officer, at a West Virginia Division of Motor Vehicles regional office. For help, call the DMV CDL Unit at 304-926-3801.

• You have been licensed for at least two years on a military vehicle similar to the commercial vehicle class you are applying for
• If you are a former service member, you were honorably discharged within six months of your application
• You have not held more than one licence, other than a military licence
• You have not had any licence suspended, revoked or cancelled
• You have not been convicted of a disqualifying major offence or of more than one disqualifying serious traffic violation in any type of motor vehicle
• You have not been convicted of violating any military, state or local traffic control law as a result of a traffic accident, and have no record of an accident in which you were at fault
