The WDVA Veterans Benefits Resource Center will help you or your family find copies of your DD214, your military records, or your service medals.

Mail, email or fax the WDVA Request for Release and/or Discussion of Veterans Records and Issues to WDVA, 2135 Rimrock Road, PO Box 7843, Madison WI 53707-7843, email wisvets@dva.wisconsin.gov, fax 608-264-6089. For help, call 800-WIS-VETS (800-947-8387).

What you'll need

Whatever service details you have, such as the veteran’s name, service dates and branch, plus proof of your relationship to the veteran if you are a family member.

Eligibility

  • You are a veteran or the family member of a veteran
  • You need a copy of a DD214, military records or service medals
