This plate is for veterans, service members, and service academy students and alumni. You can order it with two decals recognising your branch of service, wartime service, medals you received, or the service academy you attended, chosen from more than 50 designs. You can personalise the plate for $15 a year, and it can be transferred to a surviving spouse.
Mail your completed military licence plate application with your proof of eligibility to Wisconsin Department of Transportation, Special Plates Unit, PO Box 7911, Madison WI 53707-7911.
A copy of your DD214 or equivalent, official documentation that you attend or graduated from a service academy, or certification from your commander if you are currently serving.
