Your state active duty or training orders, a line of duty determination, your medical records for the injury, and a death certificate if you are a survivor filing a claim.

Eligibility

  • You are a Maryland National Guard service member
  • You were injured, became ill or became disabled while serving on state active duty or conducting authorized training, or you are the survivor of a service member who died doing so
