§ 4.71a Schedule of ratings—musculoskeletal system.

Knee limited extension (straightening)

The knee has reduced ability to straighten.

What the rating means

VA rates limited knee extension under diagnostic code 5261, called "Leg, limitation of extension of" in the rating schedule. The rating is based on how far you can straighten your knee — measured in degrees — where zero degrees means the leg is fully straight. The less you can extend your knee, the higher the rating.

A 0% rating applies when extension is limited to 5°. If extension is limited to 10°, the rating is 10%. Extension limited to 15° is rated 20%, and extension limited to 20° is rated 30%. If you can only extend to 30°, the rating is 40%. The highest rating under this code, 50%, applies when extension is limited to 45°. The difference between each level is the specific degree at which your knee stops straightening, as measured on examination.

Evidence that helps

The key evidence for this condition is a measurement of how many degrees short of fully straight your knee can go. A VA or private medical examination that includes range-of-motion testing — specifically recording the endpoint of active and passive extension — directly shows which rating level applies. Multiple measurements taken during the same exam, including any noted pain on motion or fatigue with repeated use, help establish an accurate picture of your limitation.

Treatment records such as imaging, surgical notes, and clinical progress notes can support the history and severity of your knee condition. Your own statements, and statements from people who know you, describing how the limited extension affects your ability to walk, climb stairs, stand for periods of time, or perform your job can help show the real-world impact. If your extension varies day to day, records documenting your condition on different dates may reflect the range of your limitation more completely.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
